At the end of the 90 days, the two of you must marry or she must return to her country. If you marry within the 90 days, she may remain in the country as your wife and she does not need to return to her country. You will need to file for Adjustment of Status immediately after you are married. Your new wife will not be able to leave the US until you file for "Advance Parole" or travel permission from the INS.
